  • Download VA Form 21-0966 (Intent to File).
  • Fill basic info and check the type of benefit you intend to claim.
  • Submit online via VA.gov, by mail, or at a regional office. Keep a copy.

  • Service records and your DD214
  • Medical records (VA & private), test results, treatment notes
  • Nexus letters (doctor linking condition to service), if available
